What is her exercise routine? You can change her diet all you want. But if she is frail/small w/ low BMI, she must have metabolically active lean mass/tissue that can utilize the diet intake.
She needs to walk every day and lift weights 2-3x a week. Yoga, tai chi, etc.

Ezetimibe has a half-life of ~22 hours. It's pretty long. My 5 minutes of thought on this would be to take Omega-3 fatty acids in the morning with coffee (5-7 AM), take the Ezetimibe at 10 AM and have my first meal at 11 AM - 12 PM. Never heard of this. Closest thing I'm aware of is Yadong Huang, MD, PhD at Gladstone working on ApoE4 structure correctors to make E4 work like E3.

I would think they should take interest in your sdLDL, HDL and the totality of an advanced lipid profile. Often hard to know exactly what's going on with a person just through a basic LDL figure in isolation.
