Julie, Yes, hydration can play a part. I'm in the process of moving and Kensey's book is in a box somewhere. I've seen enough n=1 stories , both online and among people I know, where people have high LDL-P and no calcification or progression to question whether a high LDL-P in someone with a VLC diet is the same. T is on Dr. Bredesen's list to optimize. My friend who is big into bio-identical hormones says you must use a) bioidentical hormones and b) test frequently and carefully for dosage.

BTW, my meals do have carbs - like salads with 15 different items that are non-starchy & mostly leafy.

An example of my experience. I hand milled (rolled) organic whole oat groats and cooked them for oatmeal, with no additives except water. It was long ago that I did this, so don't have the data handy, but my serum glucose spiked to 140-170 mg/dL on this "healthy whole food."
