I can see how, if someone has been diagnosed with MCI or Alzheimer's that a ketogenic diet might help (assuming impaired glucose utilization). But, is there any evidence that following a ketogenic diet for cognitively healthy individuals may reduce risk?

Thanks!

One more on evidence ketones can help prevent Alzheimer's. Just published (May 31, 2024) in Neuroscience News Ketones May Help Reverse Cognitive Decline
Summary: As we age, our brain naturally becomes more insulin resistant, disrupting neuron communication and leading to cognitive decline and neurodegeneration.

Researchers studied how acute insulin resistance impacts neuronal function before symptoms of chronic conditions like Alzheimer’s manifest. Using mice models, they found that ketones could restore impaired synaptic activity, axonal conduction, and network synchronization.
The article is based on this paper: D-ꞵ-hydroxybutyrate stabilizes hippocampal CA3-CA1 circuit during acute insulin resistance by Nathan A. Smith et al. PNAS Nexus, 16 May 2024

Hi Kikiwebb, people I know following a KetoFlex diet do not alternate one day of keto then one day of carbs.
That is not what the “Flex” of KetoFlex means. Unlike some ketogenic diets where no carbs are consumed, with plant-based KetoFLEX, some carbs are eaten as part of whole foods .
FLEX in KetoFLEX refers to metabolic flexibility, the ability to use either glucose or fat as a fuel source. If glucose from carbs is available, the body won’t switch to burning fats. With low carb intake, once your body burns through any available glucose, it will switch to fat-burning mode.
If you are alternating days between keto and carb-fueling, that is usually called “cycling out of ketosis”. That is a strategy that is employed for people who lose too much weight on the KetoFLEX plan, since it is important to maintain a healthy BMI. Even for those individuals, it is not one day on/one day off. More likely they will have higher carbs one or two days a week to stabilize their weight, though what I’ve observed is that most people who add enough healthy fats to their diet can maintain a healthy weight without cycling into higher carb days.
