Hi, I am a 48 year old mom.
I lost my mom to Alzheimer’s last year at age 77, she was Apoe4/4, her symptoms started at age 55 shortly after going off HRT. Her dad, my grandpa, also had Alzheimer’s, it was shown in his autopsy from car crash at age 77.
I was recently tested and learned I too am Apoe4/4. I am having word finding issues, difficulty sleeping through the night and night sweats as I am going through perimenopause transition. I am researching hormones/HRT, my added complication is that I am MTHFR homozygous C667T, subject to clotting, stroke etc. My OBGYN, a D.O. recommended I use sublingual bio identical Progesterone, but she will not prescribe it for me because of the clotting risk. I am desperate to figure out a progesterone dose and a safe provider of over the counter progesterone to help preserve cognition and ease my perimenopausal symptoms. I am struggling to find any recommendations for HRT for APOE4/4 and homozygous MTHFR mutation. I am hopeful to gain some insight here. In the meanwhile, I am working on implementing RE-Code and am entering my 4th week of nutritional ketosis. Thank you.

I am a support intern and I warmly welcome you to our site and community.
I am so sorry you lost your mom last year. I also lost my mom to Alzheimer's last year too so I more than empathise and feel your pain. I often think that the best way to pay tribute to her is to learn about this disease in order to not follow in her footsteps and you have taken some great strides already! Well done on being in your fourth week of ketosis - that is an amazing achievement (one I haven't matched!). ReCode is an incredibly powerful protocol and I am sure you will start to feel the benefits.
In terms of your questions on hormones and progesterone, I am attaching some links and I am sure others on the site will share their expertise.
Here is a link to a very long thread on HRT
Here are posts using the Search terms HRT and homozygous MTHFR in the Search text box below your username: Search
One of those is this specific post from 2017: HRT and homozygous MTHFR
Once you've got some answers on hormones, you might want to explore some other topics on the site.

Hi. A medical professional trained in ReCode would be a big help in navigating all of these issues and focusing your efforts. I have one specific suggestion re the sleep. I wish that I had had Dock Pro when I was going through the transition. I am past all the hormonal swings now, and even so, I love this thing. It is amazing. I sleep on 78 degrees, never wake up sweaty. When my body cools off in the early morning and I want warmth, I pull the comforter over me. Warmth from above, coolness below.IslandMom1 wrote: ↑Mon Sep 09, 2024 7:52 am difficulty sleeping through the night and night sweats as I am going through perimenopause transition.

Re: New here? Some Best Practices
I am providing an update to my opinion above in case anyone is thinking about buying Dock Pro based upon my 9/9 post. I just purchased the me system b/c my husband doesn't like the product. The me system is 1/2 of the bed. It arrived Fri and we set it up yesterday. It worked for 10 minutes. While it was running, there was a distinct high-pitched whine. I was hoping that would resolve. This seems louder than the previous whole bed system. Then the whole thing turned off. I could not get it to turn on. I resigned to sleeping hot last night. The bed pad does not feel good when the cool water is not running through it (why my husband does not like it). I woke up 2 hours later so hot and remembered that I have a small fan. Got the fan. When I plugged it in, my hand passed over the dock pro unit, which was wicked hot! Even though it was off. Scary. I unplugged the unit.
Customer service has responded to my emails saying that we need to set up a time for tech support to call to troubleshoot. I responded that I am very familiar with this product - it is my third time buying from them - and I don't want a unit that has already overheated after its first use. We will see what happens.
I checked reviews on Amazon, and this is all very consistent with many one-star reviews. I have lost trust. The company was recently acquired. I will almost certainly send this back. Quality control seems non-existent. An electrical device that overheats while it is off combined with a water system on the bed and many people who have had the same experience...what could go wrong?
I will follow up with something from here

And if it turns out that this all is resolved in an amazingly good way, I will update this thread. I am posting immediately because I don't want someone to spend all of this $$$ based upon my comment above.
Obviously, my Oura sleep score wasn't great last night.

Re: New here? Some Best Practices
Hello,
I have recently received results of genetic testing to determine my genetic risk for Alzheimers disease. My father died of Alzheimers disease some years ago. My older sister, age 81, is now experiencing some symptoms and is taking a drug to slow the progression of the disease. I am 75 and haven't noticed any symptoms of cognitive decline. The results of my test were 3/4, or one copy of the ApoE allele. Is this website an appropriate source of prevention information for me? You mention that a majority of those participating here are 4/4.
Thanks very much,
Baird

Baird, welcome. There are a lot of 3/4's such as myself. It is a perfect place to be.

Hi Baird,
I am a member of the support team here at ApoE4.info and I too would like to welcome you to the site. I am so sorry that your father and sister have been affected by Alzheimer's disease. Their experiences have no doubt taken a toll on you. At the same time, it is great to hear that you're not experiencing any signs of cognitive decline yourself. That is great news. On this site you will find a great deal of preventive information for people with ApoE 3/4, ApoE 4/4, and neither: much of the information supports an optimum approach to cardiovascular and brain health for any human regardless of their DNA! Having said that, any of the preventive protocols or habits you learn about here are particularly beneficial for individuals with ApoE 3/4 and ApoE 4/4.
It would be a great idea to start with our Primer , which is a detailed and informative resource written by a practicing MD with ApoE4/4. It offers research-based prevention strategies and will also help you to understand the relative significance of your ApoE3/4 result. This genetic makeup by no means determines your destiny. You can also find a wealth of information in the Wiki .
The How-To Guide includes topics such as navigating the forum, private messaging, and searching. One great tip is using the quote (") button when replying to a post. Using the button will automatically alert the member of your response.
You can read about other members' experiences and post your own on Our Stories. Sometimes reading the stories of others helps us realize that we are not alone.
I applaud your curiosity and self-advocacy in learning as much as you can. This site is the source of a inspiration as well as information, and a great starting place! Please don't hesitate to contact us should you have any further questions.
